If you’ve been awarded a scholarship through The Community Foundation, you will receive a profile form to be completed and returned to our office. The form provides us with the information necessary to process your scholarship payment. It also helps us honor your accomplishments in our publications. Please mail your profile to the address below or e-mail the requested information to our Program Officer.
Once your form has been returned, your scholarship payment will be sent to your chosen college or university before the beginning of classes. In the case of McDonald’s scholarship recipients, your scholarship check will be mailed to your home address.

Please remember to keep us posted if you change schools or change addresses.
